POSITION SUMMARY

) A& K2 i, c5 C$ j# HThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.3 r5 G% M9 u* ]8 Q* p0 F& I

The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.2 J; d0 _% r0 m9 H

• To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.

• To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.4 X- n7 i; J6 R7 z7 K
• To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

• To develop long-term, trusting relationships with both patients and peers in providing primary care services. . I2 P; k8 v. W/ u( A7 i a; c( H
• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.0 e( N. [$ K1 Q! d0 ]; F
• Ensure the maintenance of accurate records in line with practice policy.

Qualifications/Experience c. y2 J/ @* g6 l0 w
• Baccalaureate degree in Nursing preferred
# N' ?9 g9 r: J• Preference given to those with at least 5 years nursing experience in primary care setting.
9 O* Y4 H }0 N* e3 `: i7 i( F• Current CARNA Registration
: S$ w& N9 b) |7 W8 y) b0 v• Current CPR – Healthcare Provider; Y& q( {& {) }
• Must have own vehicle and valid driver’s license
