The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

: }0 y! ?! l: u: r) SThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.4 I9 R# b; N- I* Z0 V. J

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

Clinical Role

% Y* v' b( Q+ n7 A/ i; a4 i" H• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.
* k( O {. u8 c; t1 E9 f: v" m• To develop long-term, trusting relationships with both patients and peers in providing primary care services.

• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.4 Y7 z. v) |! h3 @& O% c
• To act as a resource to other multidisciplinary team members, physicians, and community resources.

Qualifications/Experience
9 \, U& W, c" [• Baccalaureate degree in Nursing preferred# a% O* V1 [8 F" X
• Preference given to those with at least 5 years nursing experience in primary care setting.6 f0 r' R* l5 h
• Current CARNA Registration( N7 T6 P5 \' S, y, ?1 x
• Current CPR – Healthcare Provider) M" x) g* L7 P" A' u9 D) q, _. Y+ r
• Must have own vehicle and valid driver’s license$ t. s5 z6 P& {+ q
• Knowledge of, and ability to use, electronic medical records; G8 n R% Z4 l
