: z+ \ E2 a7 S- x6 [3 nThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.- i7 b! k7 O5 K( c5 [
, ^: @/ f) l7 l* S, MThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

Qualifications/Experience
' E! e. R" L; i+ F• Baccalaureate degree in Nursing preferred0 l7 C9 _1 y4 j, V
• Preference given to those with at least 5 years nursing experience in primary care setting.) C0 n" ~3 ^, U6 Y3 K1 @3 {. X
• Current CARNA Registration
1 ?4 G# V% b( J• Current CPR – Healthcare Provider C" x" R4 j+ m( Y
• Must have own vehicle and valid driver’s license! E* P6 n* U/ ]2 p4 h0 O
• Knowledge of, and ability to use, electronic medical records
