1. sub floor did not hold tight to the floor joists. Screws are doing better than nails for this purpose.. F7 C; r9 L/ C- x, [
2. supporting beams are a bit too far away from each other.

For 2, you may need someone with engineering knowledge to find out, but there should be a building code governing the span distance. For 3, you can go down the basement to do a visual check yourself.
