! e8 D: O6 J1 @4 T8 u- \community cultural brokers from New Canadian Health Network partner organizations to
( S5 {" {2 O+ u, B* v0 wprovide supplemental support to newcomers who have complex needs.

" S$ f/ l0 y9 V% Q _; t/ u8 z Nature of Work: The Social Worker will report to the manager of mental health services.
" Q" L9 L$ a* C3 |2 {4 \+ b9 dThe position builds working relationships with New Canadian Health Network partners’ staff, & s2 o1 q/ h7 J% l& y
newcomers accessing these services and other institutions and service providers. The Social
- ?0 k! E0 Z VWorker will assist in case planning, conferencing and providing support and advocacy as 9 V+ T* ~6 G" P' [5 b& ~
needed. As required, the Social Worker will assist in accessing community supports for g$ e& o5 |1 h5 g' g; {
newcomers and their families. The Social Worker provides holistic and flexible initial
- ~6 h2 A* f7 A! cassessment of life circumstances, needs, goals and aspirations and adheres to the client-4 C+ x2 ^( t4 s$ G& w1 m6 Z

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental

1 B o$ \( S; m4 O7 S" ^6 F( w Relevant undergraduate degree or diploma, or equivalent combination of education and 8 N& V3 Y) D/ r# N$ y' d! x
experience in Social Work or related human service fields.
p7 ^. l) c; E% r Extensive knowledge of community resources, including in depth understanding of family

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
