Primary purpose: The Social Worker works in partnership with settlement workers or
0 R! X* S0 ]' a* jcommunity cultural brokers from New Canadian Health Network partner organizations to
# v. z" G2 a& b) H! V: M0 Fprovide supplemental support to newcomers who have complex needs.

( U6 z' ]+ x) ]- ? Nature of Work: The Social Worker will report to the manager of mental health services.
- T. o% U! r3 J+ R4 ?9 c% _The position builds working relationships with New Canadian Health Network partners’ staff,
( i7 ^6 Z: | y0 x1 Snewcomers accessing these services and other institutions and service providers. The Social
4 A5 w$ b( v! E6 q" @Worker will assist in case planning, conferencing and providing support and advocacy as 9 e- _- X I1 w, a
needed. As required, the Social Worker will assist in accessing community supports for
3 f9 |+ f* P4 L, i8 ?newcomers and their families. The Social Worker provides holistic and flexible initial
$ ?" R: @# u* N. Tassessment of life circumstances, needs, goals and aspirations and adheres to the client-
5 V% C% x) B7 |centered delivery model.

Work in partnership with settlement workers or cultural brokers to provide supplemental

Qualification
5 a( t" ?( t# M
0 d. ?) N8 g' n$ @ Relevant undergraduate degree or diploma, or equivalent combination of education and
8 ]3 X0 T9 D3 }) zexperience in Social Work or related human service fields. / }# `# x2 C) y- R4 h$ A |7 O
Extensive knowledge of community resources, including in depth understanding of family
; F1 T: F$ j! u: K4 o0 d2 {6 Jsupport needs
/ o( G. Q$ I# H2 J4 B$ j* n Experience/facility with MS Office computer applications ' m' r5 x/ D% \$ Q5 {1 s
Proficiency in both official languages is an asset, as is fluency in other languages " D. b) A L8 P( s0 ]+ n8 U( z, k
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
