Target:        Low income families with children/youth aged 3-17 residing in Edmonton

% e+ x7 u4 W. q. |Time        9:30am – 11:30am
2 E- d$ w( ~7 m1 A3 [Place        ASSIST Community Services Centre (9653-105A Ave)
# @8 }2 z- i' b% \1 P+ IPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
9 }( g' U( D2 V& _1 V# u2 P1 [$ ]Fees        Ranges from $10 to $35 per week
* F# e, K5 f! U- N( N$ f- L* j       
9 D. W( O# E$ QRemarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
; D, D) w" r0 m1 a) q8 L9 d4. Information on Child’s swimming level when registering for swimming classes
; }' Z' t  u* S  b5. Children are not required to be present at the Registration Day
& w7 O4 w5 j% O" v! e, O# m6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

' a9 ^9 f# j' w" d' Q8 QWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
