Date        May 14, 2005 (Saturday)

' q' }! K. b8 R# y# F- F: J8 [' kPlace        ASSIST Community Services Centre (9653-105A Ave)
5 F% H6 m3 e) J/ E+ sPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
& ]1 p4 D# ]. L1 g2 i4 O       
Remarks:
5 u8 W6 g# ]) b+ a) d9 Y0 m( W1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
0 {  u: M/ e1 D  t3. Each child is limited to 1 to 3 programs per organization
$ k* P( V1 ~! [* W5 l4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
