When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic 4 H/ u2 U- t: R& j: Y+ [condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and + Z1 i' M3 @" H8 C• there have been no new symptoms or change in symptoms; and/or % Z9 S4 I4 R! f• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.- b+ w! @2 k( A- ^) O# k
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
