$ J, c$ k7 s7 @) C. OThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.' Z7 y5 o& |% Y5 R$ k. v- u

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.* \3 R; i4 w) i! R8 l: ]( b7 Y% j
• Ensure the maintenance of accurate records in line with practice policy.9 w) B! b X* P& G3 u3 O/ N
• To assist in developing and implementing patient self-management programs.) z# o+ Y* n# t9 d1 E$ U
• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

SELECTION REQUIREMENTS:

Qualifications/Experience' }3 a9 S0 [& Y/ j7 L- O3 w
• Baccalaureate degree in Nursing preferred% ^1 S2 u8 W4 \+ i
• Preference given to those with at least 5 years nursing experience in primary care setting.
9 d' _8 s3 ^" ]# Y, r- R8 Y• Current CARNA Registration, \! `) ~9 o9 K" @5 |( t* g+ d
• Current CPR – Healthcare Provider8 u6 \% J& h, K) l; V- w L3 F
• Must have own vehicle and valid driver’s license
