Primary purpose: The Social Worker works in partnership with settlement workers or 6 b9 ^5 V1 H- ~$ V/ ?" w
community cultural brokers from New Canadian Health Network partner organizations to 6 r" n! q% F: ^$ T" I1 o
provide supplemental support to newcomers who have complex needs. ( L2 T! r6 |" x/ v; u

Nature of Work: The Social Worker will report to the manager of mental health services.
0 B5 z7 R5 _$ g9 |+ O7 l5 {The position builds working relationships with New Canadian Health Network partners’ staff, * V v6 Z0 J) V+ B m/ y
newcomers accessing these services and other institutions and service providers. The Social
; v. s V. k7 r5 P/ j' cWorker will assist in case planning, conferencing and providing support and advocacy as
" k9 ]/ ^# X3 q. k+ T; Lneeded. As required, the Social Worker will assist in accessing community supports for
$ s' \6 k5 S& Enewcomers and their families. The Social Worker provides holistic and flexible initial 8 k. a( d) M* c! x+ E- A
assessment of life circumstances, needs, goals and aspirations and adheres to the client-* A' L. ~" I$ d/ M
centered delivery model.

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support

Relevant undergraduate degree or diploma, or equivalent combination of education and
- {5 B& v1 b+ l( x G" nexperience in Social Work or related human service fields.

/ T1 V8 M3 g- Y1 {, r' Lsupport needs

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate

Experience: Minimum of 3 years experience working in a related field 5 j) u. }) I6 Q

Demonstrated cultural sensitivity and strong commitment to providing quality services to

Flexibility in terms of scheduling, including evenings and weekends at various locations
