Primary purpose: The Social Worker works in partnership with settlement workers or ' K7 F) E. B% r" r" |; O
community cultural brokers from New Canadian Health Network partner organizations to ( e M T- `+ a
provide supplemental support to newcomers who have complex needs. 6 S2 V8 C4 {+ N" A% }. Q0 M

Nature of Work: The Social Worker will report to the manager of mental health services.
7 ^) [ |, g+ T5 sThe position builds working relationships with New Canadian Health Network partners’ staff, ( D* K+ F9 j+ N3 {6 a
newcomers accessing these services and other institutions and service providers. The Social 0 y1 a& n& o% V( W" t
Worker will assist in case planning, conferencing and providing support and advocacy as , j- z. Q$ d& Y7 A' Q5 z. j
needed. As required, the Social Worker will assist in accessing community supports for
/ s: k% p. E+ qnewcomers and their families. The Social Worker provides holistic and flexible initial 7 J) r( X! J& Z) h! C. ]$ Z! N
assessment of life circumstances, needs, goals and aspirations and adheres to the client-0 S; v- {! ?$ k3 t
centered delivery model.

Providing holistic information or advocacy services for immigrants and refugees served.

Work in partnership with settlement workers or cultural brokers to provide supplemental 5 K( e- }( A# S9 d' O0 z. }; `
case plan support for clients who have complex needs. ' z1 Q+ l: ?) Z0 b( @. u
Advocate on newcomers’ behalf to programs, institutions and service providers. 9 O9 ?5 }2 ]+ I6 J2 k1 I2 w

Qualification ; \% B. U% h8 ^/ S; |
& g5 ~ [; M: ]+ @! s& ` Relevant undergraduate degree or diploma, or equivalent combination of education and 2 O; j3 P" W/ d" r9 h$ Y3 O
experience in Social Work or related human service fields. 6 ? r5 {, X c' t
Extensive knowledge of community resources, including in depth understanding of family 8 H) a9 p( t& [9 o5 q+ K# I# ~8 ?

Experience/facility with MS Office computer applications

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate ! `- N, P+ F) M+ \' {! p
