provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services. : K9 {6 w0 a/ N) N) q5 [7 F
The position builds working relationships with New Canadian Health Network partners’ staff,
6 V, t4 |/ s/ c3 v2 w+ inewcomers accessing these services and other institutions and service providers. The Social ! I. ~! i- W, I+ l
Worker will assist in case planning, conferencing and providing support and advocacy as
* ~6 G. m4 F/ `# _% l6 k& Nneeded. As required, the Social Worker will assist in accessing community supports for
! e4 X; |5 b! D `' O' u8 cnewcomers and their families. The Social Worker provides holistic and flexible initial
. \; H+ t5 i5 k, L$ o8 h: E cassessment of life circumstances, needs, goals and aspirations and adheres to the client-% q- F8 T7 |0 I' A
centered delivery model. . t! j' i/ t: K- {$ ^% B/ I# E

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental
" u5 w5 n; G, A# k$ C4 |case plan support for clients who have complex needs.

Relevant undergraduate degree or diploma, or equivalent combination of education and
1 i! r2 P5 }2 L. }# q& \5 y* k+ Eexperience in Social Work or related human service fields.
- [% H3 n* O+ O3 M' T4 ~4 w Extensive knowledge of community resources, including in depth understanding of family / P. Z ~8 F0 r. ]+ m8 f+ m

Experience/facility with MS Office computer applications 7 W% T6 a" F" u5 f/ d7 X$ d9 L; k
Proficiency in both official languages is an asset, as is fluency in other languages . A$ I( W5 d/ y B( o* u& P" e+ ~9 i4 z4 S
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 4 z* d5 v7 \7 W. v0 Y9 W

Experience: Minimum of 3 years experience working in a related field 8 L6 `! x) R0 k( }- k

Demonstrated cultural sensitivity and strong commitment to providing quality services to
