community cultural brokers from New Canadian Health Network partner organizations to

; H. n4 l' S/ e8 z) z) r7 ~ ^' V Nature of Work: The Social Worker will report to the manager of mental health services.
) n1 ]" w9 i" N1 ^8 y! eThe position builds working relationships with New Canadian Health Network partners’ staff,
4 {/ ]$ n. F- Z; Anewcomers accessing these services and other institutions and service providers. The Social
3 v* v% [1 S* ]) B3 R$ H' lWorker will assist in case planning, conferencing and providing support and advocacy as
4 j9 r0 E7 b7 Jneeded. As required, the Social Worker will assist in accessing community supports for # |3 j* b( b( b" i& _1 s6 |
newcomers and their families. The Social Worker provides holistic and flexible initial & t' y6 ]9 O# B9 F m" [. t8 u
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Developing of social networks and community supports for newcomer families. " j7 ~; X( a4 ~/ l0 J6 v2 T
Provide intake, needs assessment, referral, case plans and action plans to support

Advocate on newcomers’ behalf to programs, institutions and service providers.

Relevant undergraduate degree or diploma, or equivalent combination of education and

Experience/facility with MS Office computer applications

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
