Primary purpose: The Social Worker works in partnership with settlement workers or $ m. V4 w9 O: O' p* f9 H/ T& S4 T N
community cultural brokers from New Canadian Health Network partner organizations to 9 j- k% @. J. {( L, e
provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
& d" Y, q$ [1 f; }! i6 CThe position builds working relationships with New Canadian Health Network partners’ staff,
" Z3 @) E! w- Q, Q2 c; Bnewcomers accessing these services and other institutions and service providers. The Social & x& N' b2 f& D- X; {3 c9 [" m3 u3 t
Worker will assist in case planning, conferencing and providing support and advocacy as
( o8 Q6 E3 n" h3 \, v% i" e0 U, [! Hneeded. As required, the Social Worker will assist in accessing community supports for
9 R0 R* g1 V" A |, o8 Hnewcomers and their families. The Social Worker provides holistic and flexible initial 1 Z. U* T+ v7 j
assessment of life circumstances, needs, goals and aspirations and adheres to the client-( \; i0 z5 `# e- ~& P* b- K" [* t
centered delivery model. 5 I5 p* O' z8 _$ ]

Providing holistic information or advocacy services for immigrants and refugees served.

Work in partnership with settlement workers or cultural brokers to provide supplemental F. E0 r9 T0 a) y3 j/ X4 c: c
case plan support for clients who have complex needs.

Relevant undergraduate degree or diploma, or equivalent combination of education and
}, B' g& h3 |* m& W3 Z) q0 H1 n( R. wexperience in Social Work or related human service fields. % L& G9 K1 A; u
Extensive knowledge of community resources, including in depth understanding of family R) G" _- r& J5 |# z& Y* v! G
support needs % Z8 U/ g' x: r" S+ Q
Experience/facility with MS Office computer applications # o- O( B9 q- y" _5 {6 u# r
Proficiency in both official languages is an asset, as is fluency in other languages
& f* J5 I$ B" B Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
* w, D- h( C) r" Iinsurance coverage for work ! F& \: C& c( r6 V+ w4 `% ?8 h
! e0 |: g) w8 o C8 @Experience: Minimum of 3 years experience working in a related field + q. u+ c7 j! d
