Date        May 14, 2005 (Saturday)
: `$ `4 S  [& I  N$ D1 DTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
Remarks:
/ c) F, ~/ h* I: q5 m( B: D1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
3 K0 W0 q/ _5 K6 n+ P0 R7 f2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
$ Q" X6 o8 B" D/ @6 ~3. Each child is limited to 1 to 3 programs per organization
, x; @/ f7 |$ D% v  k7 Y4. Information on Child’s swimming level when registering for swimming classes

6. Registrations will be accepted on a first-come, first-served basis

0 `/ u4 d) x: X2 C# [! aWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
