Target:        Low income families with children/youth aged 3-17 residing in Edmonton

Place        ASSIST Community Services Centre (9653-105A Ave)
) P& N  q) ^' k3 x, q& [- z. G  mPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
3 J" l+ h! N/ h5 _+ Q0 R5 \1 _Fees        Ranges from $10 to $35 per week

Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
& i8 c( k. M: I0 p6 c2 x2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
" K+ j/ F( H' ~/ t3. Each child is limited to 1 to 3 programs per organization
4 F- D1 b9 r3 Z% y4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
! i% c2 k2 D% ]( }  S" L% A" G6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications
! N! j" Y# `* l& Q; B( v
0 J' A$ `/ \( }+ b0 J1 HWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
