7 s9 @% E3 ^% a/ ]Target:        Low income families with children/youth aged 3-17 residing in Edmonton
: f$ w$ @" W4 ^) iDate        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
" g: [; X# s* p2 EPlace        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
Remarks:
. ^3 d: p! |+ l7 U3 M6 B" v1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
) x5 c& I1 j# |, ^4. Information on Child’s swimming level when registering for swimming classes
. P3 J( A9 e9 X9 t0 U: m0 m+ W6 z5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

9 E3 a+ v' ^$ K8 j  UFee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
