•At any City of Edmonton leisure, sports & fitness participating facilities.

Monday-Friday except holidays, 8:30am-4:30pm.
6 x9 ?8 A' L4 g  r•By calling 780-496-4918 to have one mailed to you.
! M3 D) A# m( f: g0 q3 M4 l•For individuals under custodial care, please also submit the address verification form.
% @3 f. b' R. e8 V# @4 V•For individuals under government care, please also submit the consent for release of information form.
