6 M4 v9 o3 |8 H/ w' i9 N: ^2 h/ O+ NThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population." i6 T6 b; P( o, A

The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.8 ? w$ L/ \' |5 }+ o$ W: i, K6 R

• To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.

• Ensure the maintenance of accurate records in line with practice policy.

Qualifications/Experience
/ t' g1 K7 R. a# J. F3 Q• Baccalaureate degree in Nursing preferred
2 M3 x6 w2 N+ c7 a- h( Z• Preference given to those with at least 5 years nursing experience in primary care setting.
+ u1 k/ J4 \7 W2 g- J• Current CARNA Registration
$ D: ?6 @) t9 k2 G, E8 e• Current CPR – Healthcare Provider$ W; b5 d- s. a
• Must have own vehicle and valid driver’s license
9 U9 g3 r4 ^! d; a' {6 _• Knowledge of, and ability to use, electronic medical records
