community cultural brokers from New Canadian Health Network partner organizations to

% b3 _7 N! E9 G Nature of Work: The Social Worker will report to the manager of mental health services.
8 N3 o# v7 c0 X' P& g) ^The position builds working relationships with New Canadian Health Network partners’ staff, + Q& n, q' b: H
newcomers accessing these services and other institutions and service providers. The Social : D2 M! o' f( p' C9 \ ]1 K! k
Worker will assist in case planning, conferencing and providing support and advocacy as # o8 K% v3 y& H% `1 L
needed. As required, the Social Worker will assist in accessing community supports for 5 b/ p0 C! ^5 m# E/ O, n. y- e3 E
newcomers and their families. The Social Worker provides holistic and flexible initial - }& V8 e2 g: t$ _& R
assessment of life circumstances, needs, goals and aspirations and adheres to the client-& Y7 H( ]7 t/ h z( o& g
centered delivery model.

Extensive knowledge of community resources, including in depth understanding of family 4 T% \5 X- O B* q% S1 b0 v
support needs " `8 m, R, ]& A/ Q3 j& k: l
Experience/facility with MS Office computer applications
* @ U4 A$ M* o: s Proficiency in both official languages is an asset, as is fluency in other languages , a. V5 K: q7 r) B
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate

Experience: Minimum of 3 years experience working in a related field
