Primary purpose: The Social Worker works in partnership with settlement workers or $ x! l6 Z' ^2 D4 g
community cultural brokers from New Canadian Health Network partner organizations to 2 x$ F4 X; F8 i) g. J& x
provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
% W* @8 ?& Q$ O7 }( Y" g9 CThe position builds working relationships with New Canadian Health Network partners’ staff, 9 ~. Z, Q1 R, w
newcomers accessing these services and other institutions and service providers. The Social " }% T% i, _' Z
Worker will assist in case planning, conferencing and providing support and advocacy as
4 U4 n- {, b7 C0 Y- Hneeded. As required, the Social Worker will assist in accessing community supports for 5 S( x2 {4 z4 y1 ]
newcomers and their families. The Social Worker provides holistic and flexible initial % L) }8 n( P5 T9 S
assessment of life circumstances, needs, goals and aspirations and adheres to the client-# R- z( Z8 c* w8 H
centered delivery model. 8 n! F2 d- B$ X! k

Providing holistic information or advocacy services for immigrants and refugees served.
- }- t+ J" ] _ Developing of social networks and community supports for newcomer families.
% F- P/ r0 S) q; f g7 C5 x! V Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental 4 l0 |0 q4 ~7 X6 V+ Q7 o `, t2 o# t
case plan support for clients who have complex needs.

Qualification
0 n6 d; s# z3 d' p
5 D& }& k8 o9 Y& h3 B Relevant undergraduate degree or diploma, or equivalent combination of education and
q: @0 W" P8 f5 G5 T* W2 C/ Wexperience in Social Work or related human service fields. & Y' e1 K0 \: M+ r
Extensive knowledge of community resources, including in depth understanding of family
6 O, ^' i; P* Asupport needs 8 v# U1 I1 k, C& ~5 h; O$ e8 S
Experience/facility with MS Office computer applications ! K$ X6 [# c0 x5 v+ Y i( l5 [
Proficiency in both official languages is an asset, as is fluency in other languages 5 A% X% e4 L- T9 G5 B
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate : P4 F/ I: O k" ]
