Primary purpose: The Social Worker works in partnership with settlement workers or 5 p2 S+ r& w+ w4 F( E
community cultural brokers from New Canadian Health Network partner organizations to % I5 i3 j) G- q. r% M# N
provide supplemental support to newcomers who have complex needs. 0 ]: `. G- [+ h5 J" Q7 U

Nature of Work: The Social Worker will report to the manager of mental health services.
) T! g: Z5 ^- z3 k+ lThe position builds working relationships with New Canadian Health Network partners’ staff, * Y# Z, I5 G$ a6 Z' z& ~ E( O$ L
newcomers accessing these services and other institutions and service providers. The Social ( S) a0 N0 x9 b( n/ i0 m
Worker will assist in case planning, conferencing and providing support and advocacy as / i# A2 Y1 Q* _0 ]" n
needed. As required, the Social Worker will assist in accessing community supports for & {2 V: f( `- B S. h! `3 v4 L1 f
newcomers and their families. The Social Worker provides holistic and flexible initial

Provide intake, needs assessment, referral, case plans and action plans to support * C2 E; }/ t6 z3 V1 o/ O
newcomers in settling and fully participating in community.

Advocate on newcomers’ behalf to programs, institutions and service providers.

Relevant undergraduate degree or diploma, or equivalent combination of education and . O# M. r! z+ S4 I4 m" s
experience in Social Work or related human service fields.

support needs
; }$ n8 Z8 B" H& P4 P# Y Experience/facility with MS Office computer applications
+ }! ?& Q/ }2 K0 [! p5 g& L Proficiency in both official languages is an asset, as is fluency in other languages
