6 U' h% ~- J. T3 Q' i6 X; I1. No. 6 和 No. 11 都提到了fee payment.假如在No.6 提到的Fee payment form (IMM 5620)中选择了信用卡支付Processing Fee,那么No.11还需要提供吗?

8 u# P1 Y" P2 {  l: L2. No. 12的意思是什么? 如果勾选,是否表示用此Checklist作为Cover page,不添加其余材料?
