* Both parents, or one parent and one caregiver of infants under six months of age;
* x- t- K6 d |2 F * Children under 10 as of November 1 with chronic health conditions (e.g. diabetes);6 u, g, s; Q$ W1 Y" f4 _
* Children over six months and under five years as of November 1; and
