Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
/ C, ]. {( _4 \% A% o  l! RPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
+ i2 k0 l4 T: f+ JFees        Ranges from $10 to $35 per week
! ~2 n4 `5 V% u( [       
1 h/ m& ~7 K2 U; W7 T7 T1 H4 NRemarks:
7 ~) @! ?6 y4 ^) X+ q% S* y1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
+ X% K; Q& h- W$ R5 e3. Each child is limited to 1 to 3 programs per organization
4 T5 o$ a. l8 A- \4. Information on Child’s swimming level when registering for swimming classes
1 z9 E/ [+ R* d5. Children are not required to be present at the Registration Day

Fee Reduction Pass Applications

' @) k! f/ ?6 T7 A8 [2 [2 ?  mWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
