Primary purpose: The Social Worker works in partnership with settlement workers or + R4 w5 }* b7 b( j5 F, W
community cultural brokers from New Canadian Health Network partner organizations to

! P: {5 M) O8 G! U$ K Nature of Work: The Social Worker will report to the manager of mental health services. # J; L) ~7 R- L1 h( k
The position builds working relationships with New Canadian Health Network partners’ staff,
! v' t/ B9 W/ T+ c3 znewcomers accessing these services and other institutions and service providers. The Social . E! f/ X" r, f3 s4 _3 A$ w
Worker will assist in case planning, conferencing and providing support and advocacy as ; L8 E+ m% d5 F" K4 w
needed. As required, the Social Worker will assist in accessing community supports for
) Z" b3 R; Y6 Unewcomers and their families. The Social Worker provides holistic and flexible initial
% U! [. W n M# K& w# S% g7 ^# ]% yassessment of life circumstances, needs, goals and aspirations and adheres to the client-

Developing of social networks and community supports for newcomer families. ' T k' F+ Y" Z1 Q' ~6 o9 H
Provide intake, needs assessment, referral, case plans and action plans to support
! l; p( W4 V$ G% T; \newcomers in settling and fully participating in community.

Relevant undergraduate degree or diploma, or equivalent combination of education and
' n+ a! m% ^% l# M" ]4 wexperience in Social Work or related human service fields. : a* p: h1 l Y
Extensive knowledge of community resources, including in depth understanding of family
( ~, [/ M3 @) b4 K A' W0 usupport needs
) o% j' g+ h% x, k- p3 T Experience/facility with MS Office computer applications " H# ^& W3 T8 \2 ?* R
Proficiency in both official languages is an asset, as is fluency in other languages 1 `! z/ N/ J2 D: B% Z
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
