community cultural brokers from New Canadian Health Network partner organizations to

& ~# e7 X4 E1 p- d Nature of Work: The Social Worker will report to the manager of mental health services. " u8 V" j: r- P6 b( f
The position builds working relationships with New Canadian Health Network partners’ staff, & V0 [2 k, X$ F& |
newcomers accessing these services and other institutions and service providers. The Social $ |7 ]% [+ n6 |" `5 V+ T
Worker will assist in case planning, conferencing and providing support and advocacy as
$ w4 F7 p2 |5 {needed. As required, the Social Worker will assist in accessing community supports for * m. O/ q9 {$ Y3 M2 S
newcomers and their families. The Social Worker provides holistic and flexible initial 3 s: D& J- }! c
assessment of life circumstances, needs, goals and aspirations and adheres to the client-8 g, P& \0 {: B G; P
centered delivery model. 7 R6 h2 V1 u3 z. z( y5 B

Understands and adheres to professional ethics related to social work.

Developing of social networks and community supports for newcomer families.
! \5 y% V* m( r Provide intake, needs assessment, referral, case plans and action plans to support
o7 w% s: f6 N" `newcomers in settling and fully participating in community.

case plan support for clients who have complex needs.

/ @8 Y" U3 `8 I3 ~1 P# \ Relevant undergraduate degree or diploma, or equivalent combination of education and

9 Q( |6 ~6 I0 W4 k1 Y Extensive knowledge of community resources, including in depth understanding of family
& x% p0 Q, v0 d' e& I' tsupport needs
3 ?& s6 L+ P6 [0 X8 k Experience/facility with MS Office computer applications " I# `3 ?& c2 ]+ _
Proficiency in both official languages is an asset, as is fluency in other languages
