Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.

% o7 k! e& f& G. H1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
8 T2 |7 ?* c/ }# V2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
+ J: F0 j1 W5 n; b3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
  J* g, q3 c/ ], _. N  \5. Children are not required to be present at the Registration Day

9 O3 d0 p) |9 @% L% z& cWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
