Date        May 14, 2005 (Saturday)
  f* Y- u4 E- x9 Q) z/ xTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

Remarks:
# X8 `  z3 C) F1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
) w" b- q( r. ~. S) `  k3. Each child is limited to 1 to 3 programs per organization
  p' m$ y2 o7 p1 u4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day

Fee Reduction Pass Applications

9 F' ^& Q- {& Y# E: A4 s4 `We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
