Time        9:30am – 11:30am
0 J# S! R" c: fPlace        ASSIST Community Services Centre (9653-105A Ave)
' i) `4 P/ b  y# S$ q4 uPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
0 U& p9 W- q* A/ W, L) j. V' {+ IFees        Ranges from $10 to $35 per week
       
  ?9 z; r# n3 W' r: s  T9 V4 ERemarks:
1 r* k$ B: l! l1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
! n: G  s( Y6 s% X2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
6 o* ?) w( U. T( h1 R6 w3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
: l% n; m  D- L& [7 O' j6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

& @& ]; O* Y- H8 X' b" GWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
