Target:        Low income families with children/youth aged 3-17 residing in Edmonton

! Z2 q: h" k! Q* g# z2 fPlace        ASSIST Community Services Centre (9653-105A Ave)
; |( O% h+ U; p* {1 Z$ C$ i& R( KPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
- F  F# j2 k' R" d8 N0 K# E/ IFees        Ranges from $10 to $35 per week
       
Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
$ g- N0 d- x# ]8 V4. Information on Child’s swimming level when registering for swimming classes

6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications
* Q( U- `1 q& p4 l
4 h% M" H( i8 o: nWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
